As a new patient, you will be responsible for obtaining your prior medical records from other physicians. These records may be faxed to 270.442.2694 prior to your first appointment, or you may bring them with you on the day of your visit. Please use our Request for Release form (PDF).

Please hand-carry all vitamins, herbs, supplements and prescription drugs that you currently take.

Please fax or mail a completed Patient Information Form (PDF) and Health Questionnaire (PDF) at least one week prior to your appointment date. Please bring the original Form with you to the appointment.
